A.P.Polo - "Wuhan" - Hamburg (Germany). The world's first confirmed cases of people infected with and contracting the SARS-CoV-2 coronavirus came from Wuhan. The first cases of an unknown lung disease occurred there at the end of November 2019. From Wuhan, the virus spread around the world within months. On 23 January 2020, the city was quarantined due to the COVID-19 pandemic. The airport and railway stations were closed, local transport was suspended, curfews were imposed and residents were no longer allowed to leave the city. The curfew was lifted on 7 April 2020. In December 2019, a large number of severe pneumonia cases of unknown cause were detected in the city of Wuhan. On 30 December 2019, Chinese doctor Li Wenliang informed his colleagues in a WeChat group about seven patients who were being treated at Wuhan Central Hospital for suspected infection with the betacoronavirus SARS-CoV-1. Li himself contracted COVID-19 a few weeks later and died as a result.In addition to the coronaviruses SARS-CoV-1, SARS-CoV-2 and MERS-CoV, four other viruses from the Coronaviridae family - the two alphacoronaviruses HCoV-NL63 and HCoV-229E and the two betacoronaviruses HCoV-HKU1 and HCoV-OC43 - circulate in humans, are widespread worldwide and cause 10-15% of all colds. The infections caused by the latter four coronaviruses show seasonal patterns and usually occur in the winter months. It is assumed that these four viruses are all of zoonotic origin. The betacoronavirus HCoV-OC43 has been proposed as a potential aetiological agent of the pandemic known as the "Russian flu", which killed up to one million people worldwide around 1890 and was only surpassed by the Spanish flu, which claimed well over 25 million victims from 1918 onwards. The Chinese Centre for Disease Control and Prevention sent a team to the city on 31 December 2019. On the same day, the WHO China office was officially informed by the Chinese authorities that several people had fallen ill with severe pneumonia in Wuhan in December 2019 and that an uncharacterised pathogen was suspected to be the cause. By 3 January 2020, a total of 44 cases had been reported to the WHO, including seriously ill people. As several of those who had fallen ill had worked at the "wholesale fish and seafood market in Wuhan" - a wet market where, in addition to meat products, fish and seafood, mostly live animals or animals slaughtered shortly before sale, reptiles and many other meat products are sold - the origin of COVID-19 was suspected to be there. Shortly after the onset of the viral disease in December 2019, 27 (66%) of the first 41 hospital patients had visited the wet market in the centre of Wuhan; 13 of the remaining patients were not infected with SARS-CoV-2 in connection with a visit to the market. On 7 January 2020, the Chinese virologist in charge of virus identification, Xu Jianguo, announced that the pathogen was a previously unknown coronavirus. This was the result of analyses of blood samples and throat swabs from 15 patients. The WHO confirmed this finding on 9 January 2020, and on 13 January 2020 the complete RNA genome sequence of an isolate of the new coronavirus was deposited in the NCBI GenBank (GenBank number MN908947). Almost simultaneously, a special polymerase chain reaction (PCR) was published as the first detection method for SARS-CoV-2 - PCR is one of the most important and reliable methods for detecting viral infection - developed by Christian Drosten, Marion Koopmans, Chantal Reusken and their virology teams at Charité Berlin, Erasmus University Rotterdam and RIVM in the Netherlands. A phylogenetic analysis of the genome sequences from environmental samples from the market (e.g. from surfaces) showed that they are very closely related to the viruses of the first patients from Wuhan. According to a study by Wuhan Hospital, the first patient identified had not visited the market. None of the animals tested from the market tested positive for SARS-CoV-2, which supports the assumption that the virus did not spread to humans there. Apparently, the virus had previously established itself unnoticed among humans. The market could therefore have been the scene of an early superspreader event. Modelling based on the study of changes in the genetic material RNA of the virus indicates that the first appearance of the virus probably occurred between October and early December 2019. The spread pattern of the different distinguishable virus mutations suggests a mass global spread of the virus through a variety of different spread events. Hong Kong's largest English-language daily newspaper reported in March 2020 with reference to unpublished government data that a 55-year-old man from Hubei province could have been infected as patient zero on 17 November 2019. In a study published in August 2021, an extensive genome comparison was able to determine a presumed RNA sequence of the initial form ("progenitor": proCoV2), which (as expected) deviates somewhat from the genome of the real existing reference form. From the data, it can be deduced that this virus had already infected people several weeks before the diseases discovered in December 2019. Following discussions with Chinese doctors, the WHO estimated the number of coronavirus patients before December 2019 at around 1,000 people. In addition, 13 virus strains were isolated, not all of which can be attributed to the outbreak in Wuhan. It is possible that 72,000 earlier cases from October to December 2019 with symptoms such as pneumonia, flu or fever could have been COVID-19. However, the 92 samples subsequently tested for the virus were all negative. It is not that simple, the WHO announced in spring 2021. These cases, which were reported weeks before the first cases in Wuhan, were also being investigated to determine whether previous virus outbreaks had been overlooked, but the testing procedures were not standardised, not confirmed and there were other challenges. In August 2021, the WHO stated that its newly established Scientific Advisory Group for the Origins of Novel Pathogens (SAGO) would investigate these early samples and emphasised the importance of not apportioning blame. As of September 2021, the WHO is still investigating the origin of the virus, and the SAGO group met for the first time at the end of November. Although the World Health Organisation's cooperation with the Chinese government proved to be relatively difficult due to their unwillingness to cooperate and be transparent, the hypothesis of a laboratory accident for the origin of SARS-CoV-2 was rejected in the WHO's global study. The WHO labelled it "extremely unlikely" at a very early stage. However, WHO Director-General Tedros Adhanom Ghebreyesus later stated that the laboratory accident hypothesis had been hastily dismissed. Peter Ben Embarek, the head of the study, also explained that Chinese officials had urged him to drop the laboratory accident version. In contrast to other US authorities, the US Department of Energy and the FBI consider a laboratory accident to be the most likely origin.
