A.P.Polo - "Pandemia" - Hamburg (Germany). A pandemic is a global challenge. The term goes back to the ancient Greek word pandemia, which means "the whole people". It therefore refers to infectious diseases that affect the majority of the population. Pandemics are often caused by pathogens that are transmitted from animals to humans. Our living conditions play a role here: global trade, global mobility and environmental damage favour the emergence and spread of infectious diseases. If a pandemic spreads, we need to react quickly. A pandemic is when a disease spreads across entire regions, countries and continents. If the disease outbreaks remain limited in terms of space and time, scientists speak of an epidemic. A pandemic therefore refers to a global epidemic. The causes of a pandemic are pathogens that have not previously been present in the human population, or have not been present for a very long time. The human immune system is not prepared for these pathogens and people are therefore not adequately protected from falling ill. These are often viral or bacterial strains that were previously only found in animals. Examples of this are bird flu viruses or swine flu viruses. The genetic material of the viruses can change genetically and the viruses mutate. As a result, they can also spread to humans and in some cases cause serious illness. The plague in the Middle Ages (also known as the Black Death) was an epidemic that spread in Europe partly via the trade network of the Genoese colonies - by ship and by land. The plague is caused by a bacterium that occurs in rats and mice and is transmitted to humans via fleas. Examples of viruses are: human flu viruses, bird flu viruses, swine flu viruses, the SARS coronavirus, the MERS coronavirus, SARS-CoV-2 and the human immunodeficiency virus (HIV). According to current knowledge, the SARS (severe acute respiratory syndrome) coronavirus was probably transmitted to humans by bats. Dromedaries (one-humped camels) are considered to be the reservoir of the MERS (Middle East Respiratory Syndrome) coronavirus. The viruses were transmitted to humans directly or through another animal species (intermediate host). The HI virus probably originates from primates. Some animal diseases, such as African swine fever, are not dangerous to humans. These pathogens cannot reproduce in humans. Today, flight routes are considered to be the fastest ways for infectious diseases to spread. For example, AIDS, which is caused by the HI virus, developed from a local to a global problem, partly due to air tourism. This effect was also evident during the SARS pandemic in 2002/2003: while the classic spread routes for SARS were still assumed in Asia, the increasing number of cases in Canada clearly demonstrated this travelling effect. The spread of the 2015/2016 Zika virus epidemic in South America was also linked to intensive travel during the final round of the 2014 FIFA World Cup. The World Health Organisation has pointed out that a pandemic pathogen can overburden a country's healthcare system due to the large number of people infected at the same time. Scientists use cohort studies, computer simulations and methods of theoretical biology, such as the SIR model, to quantitatively describe and predict pandemic and similar spreading processes. In 2012, the German government hypothetically simulated a pandemic with an invented virus called Modi-SARS. In August 2019, the U.S. Department of Health and Human Services ran the Crimson Contagion simulation assuming a new flu virus from China, resulting in 500,000 deaths in the USA. Carolien van de Sandt, Peter Doherty Institute for Infection and Immunity at the University of Melbourne, and other authors warned of a second Spanish flu in October 2018 in the journal Frontiers in Cellular and Infection Microbiology. Demographic change, antibiotic resistance and climate change could make it more difficult to combat the disease, meaning that up to 150 million people could die. Chinese researchers warned of the problem of the link between bats and coronavirus in March 2019. In 1948, the World Health Organisation set up a global monitoring system that constantly checks the circulating virus strains for new variants with the help of numerous reference laboratories. These field studies form the basis for the annually renewed recommendations regarding the vaccine composition for the next winter season, whereby ten to twelve months can pass between the recommendation and the provision of the vaccine in sufficient quantities by its manufacturer. In 1999, the World Health Organisation also required all countries to draw up national pandemic plans. On the one hand, these measures are intended to ensure that safe vaccines can be produced quickly in the event of a pandemic; the Vaccine Expert Group of the European Medicines Agency has drawn up corresponding guidelines for Europe. On the other hand, a tried-and-tested catalogue of measures is intended to help break chains of infection through social distancing, among other things, and ensure patient care. Between 2006 and 2015, global vaccine production increased from an estimated 0.5 to 1.5 billion doses per year to almost 6 billion doses. Robust, collaborating public health systems that have the capacity for active surveillance for early detection of cases and to mobilize their health care coordination capacity may be required to be able stop contagion promptly. After an outbreak there is a certain window of time during which a pandemic can still be stopped by the competent authorities isolating the first infected and/or fighting the pathogen. A good global infrastructure, consequent information exchange, minimal delays due to bureaucracy and effective, targeted treatment measures can be prepared. In 2012 it has been proposed to consider pandemic prevention as an aspect of international development in terms of health-care infrastructure and changes to the pathogen-related dynamics between humans and their environment including animals. Often local authority carers or doctors in Africa, Asia or Latin America register uncommon accumulations (or clusterings) of symptoms but lack options for more detailed investigations. Scientists state that "research relevant to countries with weaker surveillance, lab facilities and health systems should be prioritized" and that "in those regions, vaccine supply routes should not rely on refrigeration, and diagnostics should be available at the point of care". Two researchers have suggested that public health systems "in each country" need to be capable of detecting contagion early, diagnosing it accurately, implementing effective disease control measures, and fully collaborating with the relevant international authorities at each stage (see below). U.S. officials have proposed a range of reforms to international health regulations and global institutions for global health security. The "entire architecture of the response to epidemics" may need to get adapted, evolving "from crisis response during discrete outbreaks to an integrated cycle of preparation, response and recovery".
